How do I start services for my child at MOST/DFW?

Please complete our Request for Service Form or contact us. We will gather your information, verify your health care coverage benefits and coordinate a referral with your primary physician.


What information is needed for referral?

To start, we need the diagnosis of the child, applicable to Speech-Language Pathology, Occupational Therapy and/or Physical Therapy, parent contact information and insurance card/policy information.


What is the referral process like?

First, an Intake Coordinator electronically submit verification for insurance benefit. Then, parents complete our intake form. Once all of this is completed, evaluation(s) will be scheduled.


How long does it take to start therapy?

Typically, the referral process takes a minimum of two weeks to start any service(s). This greatly depends on how quickly insurance and intake information is provided. All services and evaluations must be approved by your health insurance provider prior to starting therapy services.


Is my child old enough/too old for therapy?

MOST/DFW accepts patients of all ages!


What happens after the Evaluation?

After the therapist scores and completes the evaluation, we will reach out for a post-evaluation consultation, where the evaluating therapist will explain the results and make recommendations for therapy. Then, we get a doctor’s orders to complete the recommended therapy and obtain any necessary authorizations from your insurance company. Once these steps are complete, you will be contacted to schedule treatment.


Do you accept my child’s insurance?

MOST/DFW is in network with BlueCross BlueShield, Humana, Medicaid, UHC, UMR, Optum, Childrens, Aetna Better Health, Friday and Parkland.

Depending on your plan, out of network services may be available.

We we accept the following insurances: BCBS/Anthem PPO and HMO, Aetna, UHC, UMR, Optus, AllSavers, Humana, Meridian, ChampVA, and Boon Chapman. We accept Tricare out of network through 12/31/2023. Medicaid Provider through Aetna Better Health and Parkland.
